Notice: Faculty and staff enrolled in courses at Pacific are required to comply with immunization requirements

Faculty and staff enrolled in 1 or more units at Pacific are required to complete the University's immunization requirements, as you also are considered a student. Please check your immunization record at MyHealth@Pacific.

The portal is located at the Pacific Health Services website: http://www.pacific.edu/healthservices.

A green checkmark means you have completed the item.  A red "x" next to an item means that you have not yet completed it. Information about the requirements can be found on the website under New Student Entrance Requirements and Forms and Immunization Compliance. Please feel free to send a secure message through MyHealth@Pacific or call 209.946.2315, option 1, if you have any questions.

The deadline for immunizations is November 15, 2014.
