Speech Language Pathology

In preparation for a Preliminary Speech-Language-Pathology Credential:

1.      Activate a credential file by submitting the following to the credential office:

  • Email credential.office@pacific.edu requesting the "Request to Activate a Teaching Credential" form, type "Speech Language Pathology Student" in the subject line
  • Return completed form & $30 payable by exact cash or check made out to UOP
  • Certificate of Clearance, Teaching Credential or Emergency Credential

  • Copy of passing CBEST score

2.      Following completion of the program the following steps must be satisfied:

  • We must receive completed paperwork signed by Dr. Hanyak with Praxis scores & transcripts attached
  • Complete the top portion of the CTC 41-4 application found at: www.ctc.ca.gov/credentials/leaflets/414.pdf and submit to credentialing office either in person or by email to: credential.office@pacific.edu
  • Contact Student Accounts and request that they let us know you are financially clear with the university
  • Degree must post in the system - usually 2-4 weeks after graduation date
  • After steps 1-4 are complete, recommendation will be submitted to the CTC
  • You will receive a "donotreply" email from the CTC with directions for completing your portion and paying for your credential
  • The CTC will take 5-10 working days to review your application and then notify you when the credential has been issued
