Health Requirements & Forms

PLEASE NOTE: Beginning Fall 2013, the immunization policy will be strictly enforced. All Graduate Health Science  and undergraduate Athletic Training, Speech-Language Pathology and Dental Hygiene students must be in compliance by November 15, 2013 or will be assessed a $100 late fee and be withheld from clinical rotations. If the matter is not resolved within 45 days, a hold will be placed on his/her student account. All undergraduate and non-Health Science Graduate students must be compliant with the immunization requirements by January 24, 2014 or a $100  late fee will be assessed. If the matter is not resolved within 45 days, a hold will be placed on his/her student account.

 Minors - 17 years old after August 27, 2012 (required parent signature)

Housing Meningococcal Form (required if living on campus or a freshman)

Consent to Get Medical Records from Health Services (cannot be a blanket release)

 Influenza Refusal Form
