Release of Information Authorization

If you would like the University of the Pacific to discuss details of your loan account with someone other than yourself, complete the  Release of Information Authorization Form and return it to us.

Please note: You may revoke permission at any time by providing written instruction to our office.

The Family Educational Rights and Privacy Act (FERPA), also known as the Buckley Amendment, is a Federal law that protects the privacy of student records. The law applies to all schools that receive funds under an applicable program of the U.S. Department of Education.
