Immunization Compliance

University of the Pacific requires you to be appropriately immunized as a non-academic requirement of enrollment. To view your current status, log-in to MyHealth@Pacific and click on the Immunizations link on the left. Any items in red have not been submitted. If you feel that you have submitted a medical record that has not been entered, please send an email to medical records to review your information. To submit medical documentation to fulfill your requirements, you must submit them by mail or in person. Faxed documents are not accepted. FOR SECURITY AND PRIVACY REASONS DO NOT EMAIL US YOUR MEDICAL RECORDS

This policy is in place:

  • To protect the Pacific community from outbreaks of infectious diseases
  • Meet the requirements of some academic programs
  • Ensures Pacific is meeting the gold standard of care set forth by the CDC, State of California and other health organizations.

Deadline:

  • Health Science students plus Athletic training students (Juniors and Seniors) must have all requirements turned in by November 15, 2013
  • Undergraduate and graduate students (non-health science) must have all requirements met November 15, 2014

Late Fee:

  • If you do not comply with the policy, you will be subject to a $100 late fee each semester that you are not in compliance. If you are in progress, (e.g. completed 1 Varicella vaccine but can't get the second one yet due to administration guidelines) you will be considered pending and will not be charged the late fee.
  • Health Science students not compliant by November 15, 2013 will be charged a $100 late fee. If you are not compliant by February 15, 2014 you will be assessed an additional $100 late fee.
  • All other students not compliant by November 15, 2014 will be charged a $100 late fee. If you are not compliant by February 15, 2015 you will be assessed an additional $100 late fee.

Appeals:

  • If you feel that you have been charged a fee erroneously, you must complete the APPEAL FORM within 10 days of the fee being assessed and submit it to the Medical Records Office via:
       -Fax to 209.946.3189
       -Email it to Medical Records
     
      -Deliver the completed form in-person to Pacific Health Services
  • Recommended browsers: Internet Explorer and Mozilla Firefox. Chrome and Safari may not display the form correctly.
  • The appeal will be reviewed by the Director of Health Services. 
  • All further communication will occur through MyHealth@Pacific via secure messages.

  • Athletes 

    View your current status online via MyHealth@Pacific. To submit new records, bring them in person or mail them to Pacific Health Services.

    • Hepatitis B Vaccine (Hep B): 3 vaccines or titer required
    • History and Physical Exam: required before practice, includes Complete Blood Count (CBC), Urinalysis (UA) and EKG
    • Influenza Vaccine: recommended annually
    • Measles, Mumps, Rubella Vaccine (MMR): 2 vaccines or titer required
    • Tetanus, Diphtheria, Pertussis Vaccine (Tdap): 1 vaccine in the last 10 years required
    • Tuberculosis Screening: complete the questionnaire via MyHealth@Pacific, after answering the questions, check your immunization compliance window to see if you need a PPD test. If you have ever had a positive PPD, you will need a chest x-ray
    • Varicella Vaccine: 2 vaccines or titer or documented disease by a medical provider required
  • Athletic Trainers 

    View your current status online via MyHealth@Pacific. To submit new records, bring them in person or mail them to Pacific Health Services.

    • Chest X-ray: required for a positive screening questionnaire and Tuberculosis Test or per clinical requirement
    • Hepatitis B Vaccine (Hep B): 3 vaccines or titer required
    • History and Physical Exam: recommended (Print and complete the  form)
    • Influenza Vaccine: vaccination or  declination form  required annually
    • Measles, Mumps, Rubella Vaccine (MMR): 2 vaccines or titer required
    • Tetanus, Diphtheria, Pertussis Vaccine (Tdap): 1 vaccine in the last 10 years required
    • 2-Step Tuberculosis Test (PPD): required after March 1 of sophomore year, prior to August 1 of junior year
    • 1-Step Tuberculosis Test (PPD): required annually per clinical rotation
    • Varicella Vaccine: 2 vaccines or titer or documented disease by a medical provider required
  • Dental Hygiene 

    View your current status online via MyHealth@Pacific. To submit new records, bring them in person or mail them to Pacific Health Services.

    • Chest X-ray: required 6 months before the start of the program if there has been a positive Tuberculosis Test
    • Hepatitis B Vaccine (Hep B): titer required
    • History and Physical Exam: required one time as a student at Pacific (Print and complete the  form)
    • Influenza Vaccine: annual vaccine recommended
    • Measles, Mumps, Rubella Vaccine (MMR): 2 vaccines or titer required
    • Tetanus, Diphtheria, Pertussis Vaccine (Tdap): 1 vaccine in the last 10 years required
    • 2-Step Tuberculosis Test (PPD): required within 3 months of the start of the program
    • 1-Step Tuberculosis Test (PPD): not required
    • Varicella Vaccine: 2 vaccines or titer required
  • Dental Students 

    View your current status online via MyHealth@Pacific. To submit new records, bring them in person or mail them to Pacific Health Services.

    • Chest X-ray: required for a positive Tuberculosis Test within 6 months of the start of the program
    • Hepatitis B Vaccine (Hep B): titer required
    • History and Physical Exam: required within 3 months of the start of the program (Print and complete the  form)
    • Influenza Vaccine: recommended annually
    • Measles, Mumps, Rubella Vaccine (MMR): 2 vaccines or titer required
    • Tetanus, Diphtheria, Pertussis Vaccine (Tdap): 1 vaccine in the last 10 years required
    • 2-Step Tuberculosis Test (PPD): required within 3 months of the start of the program
    • 1-Step Tuberculosis Test (PPD): required per clinical rotation
    • Varicella Vaccine: 2 vaccines or titer required
  • Non-Health Science Students

    • Chest X-ray: required for a positive screening questionnaire and Tuberculosis Test
    • Hepatitis B Vaccine (Hep B): 3 vaccines or titer required
    • History and Physical Exam: recommended (Print and complete the  form)
    • Influenza Vaccine: recommended annually
    • Measles, Mumps, Rubella Vaccine (MMR): 2 vaccines or titer required
    • Tetanus, Diphtheria, Pertussis Vaccine (Tdap): 1 vaccine in the last 10 years required

    • History and Physical Exam: required, must be completed no earlier than April 1 prior to the start of the Pharm-D program (Print and complete the  form)
    • Influenza Vaccine: annual vaccine or  declination form required
    • Measles, Mumps, Rubella Vaccine (MMR): 2 vaccines or titer required
    • Tetanus, Diphtheria, Pertussis Vaccine (Tdap): 1 vaccine in the last 10 years required
    • 2-Step Tuberculosis Test (PPD): required annually. Complete no earlier than April 1 before the start of the program. TSPOT or Quantiferon are not acceptable substitutes
    • 1-Step Tuberculosis Test (PPD): per clinical requirements
    • Varicella Vaccine: 2 vaccines or titer required
  • Physical Therapy
