Immunization Compliance

See below for a list of the requirements by area of study. If you are looking for Health Services forms, please go to the Forms page.

University of the Pacific requires you to be appropriately immunized as a non-academic requirement of enrollment. To view your current status, log in to MyHealth@Pacific and click on the Immunizations link on the left. Any items with a red X have not been submitted or are incomplete. If you feel that you have submitted a medical record that has not been entered or are a distance/online learner and should not have to meet the requirements, please send an email to medical records to review your information. To submit medical documentation to fulfill your requirements, you may submit them using MyHealth@Pacific under the Immunization Upload tab. Supported file types are JPG, JPEG, GIF, and PNG. Files cannot be larger than 4MB. You may also submit them in person at the Cowell Wellness Center or mail them to Pacific Health Services at 3601 Pacific Ave Stockton, CA 95211 Attn Immunization Compliance. Faxed documents are not accepted. FOR SECURITY AND PRIVACY REASONS PLEASE DO NOT EMAIL US YOUR RECORDS.

*NEW* You may now upload immunization records directly to your medical record using MyHealth@Pacific. Go to the Immunization Upload tab on the left once you have logged in.

This policy is in place:

  • To protect the Pacific community from outbreaks of infectious diseases
  • Meet the requirements of some academic programs
  • Ensures Pacific is meeting the gold standard of care set forth by the CDC, State of California and other health organizations.

Deadline:

  • Undergraduate and graduate students (including health science) must have all requirements met by October 15 of the fall term and February 15 of the spring term.

Penalty:

  • If you are in one of the following programs and do not comply with the policy by the deadline, you will be subject to a $50 penalty fee that must be paid directly to Pacific Health Services: Pharmacy, Physical Therapy, Dental Hygiene, Speech Language Pathology, Athletic Training, and Music Therapy. If you are in progress, (e.g. completed 1 Varicella vaccine but can't get the second one yet due to administration guidelines) you will be considered pending and will not be charged the late fee. You will only be charged one late fee in your Pacific career.
  • All students who do not complete the requirements by the deadline will be subject to a registration hold that will prevent you from registering or adding/dropping courses, obtaining official transcripts, and receiving a diploma. If you are in progress, (e.g. completed 1 Varicella vaccine but can't get the second one yet due to administration guidelines) you will be considered pending and will not have a registration hold placed on your account.

Appeals:

  • If you feel that you have been charged a fee or were placed on hold erroneously, you must send a Secure Message using MyHealth@Pacific. Click messages on the left. Select new message. Click the radio button to either request an appeal for the penalty fee or ask for a hold to be released.
  • The appeal will be reviewed by the Director of Health Services. 
  • All further communication will occur through MyHealth@Pacific via secure messages.

    Matriculation Requirements

    • History and Physical Exam: recommended (Print and complete the  form)
    • Hepatitis B Vaccine (Hep B): 3 documented vaccines OR positive titer results
    • Measles, Mumps, Rubella Vaccine (MMR): 2 documented vaccines OR positive titer results
    • Varicella Vaccine: 2 documented vaccines OR positive titer result OR medical documentation of disease
    • Meningitis: one vaccine adminstered after the age of 16 or  signed disclosure statement
    • Influenza Vaccine: Annual vaccination recommended
    • Tetanus, Diphtheria, Pertussis Vaccine (Tdap): Vaccination within last 10 years, updated Td if no Tdap was administered more than 10 years ago
    • Tuberculosis Clearance:
          - Online Questionnaire via MyHealth@Pacific
               - Upon completion check your immunization status to see if you need a TB test
          - History of positive PPD:
               - Chest X-ray, Quantiferon Gold, or T-Spot: required within 6 months prior to the start of the classes
  • NCAA Athletes

    Matriculation Requirements

    • History and Physical Exam: required before practice, includes Complete Blood Count (CBC), Urinalysis (UA) and EKG
    • Hepatitis B Vaccine (Hep B): 3 documented vaccines OR positive titer results
    • Measles, Mumps, Rubella Vaccine (MMR): 2 documented vaccines OR positive titer results
    • Varicella Vaccine: 2 documented vaccines OR positive titer result OR medical documentation of disease
    • Meningitis: one vaccine adminstered after the age of 16 or  signed disclosure statement
    • Influenza Vaccine: Annual vaccination recommended
    • Tetanus, Diphtheria, Pertussis Vaccine (Tdap): Vaccination within last 10 years, updated Td if no Tdap was administered more than 10 years ago
    • Tuberculosis Clearance:
          - Online Questionnaire via MyHealth@Pacific
               - Upon completion check your immunization status to see if you need a TB test
          - History of positive PPD:

  • Athletic Trainers
