Dear Pacific faculty,

In response to inquiries regarding international travel for university business, we would like to provide you an update on the approval process.

Should you wish to travel internationally for university business, we ask that you complete this form and return it to your dean or vice president for review and approval.

Upon return from your trip, you will be required to complete COVID-19 surveillance testing at one of our campus testing sites, regardless of your vaccination status.
