NOTE: Pacific employees, including all faculty, staff, and student staff (except those designated in the Policy as confidential), are required report to the Director of Equitable Rights & Responsibilities via this form, if they receive notice of an incident of identity-based misconduct.
Anyone who has been the target of, or witnessed, bias or hate may use this online form to report an incident on any of the three Pacific campuses.
