Below is links to fillable PDF  forms for commonly used Registrar service requests. Please use your pacific email account to submit a form as an attachment to registrar@pacific.edu.

Important Note: Effective March 16th 2020, in response to COVID-19 remote services, email approval will be accepted for form submission. As noted in each form, please include email approval from the appropriate approver in your submission.

If the form you are seeking is not listed above, please (1) check insidePacific under your Student or Faculty/Staff resources to find the form or resource you need or (2) email registrar@pacific.edu.

Faculty Only Forms

Faculty Only forms can be found via  insidePacific. Click on the Academic Tab to view the Office of the Registrar Faculty Channel to access the following forms.
  • CAPP Confirmation Form (Degree Evaluation Form)
  • Incomplete Grade Contract
